Gaining reflective feedback from patients with significant care needs post-discharge from hospital is challenging, creating gaps in understanding. This report aims to improve understanding by examining on pathway 0: patients with no care needs upon discharge, focusing on the Queen Elizabeth Hospital in particular.
Our findings suggest significant variations in the patient care experience, especially when it came to communication and involvement in decision-making. In particular, a lack of post-discharge follow-up was noted. Half of the participants requiring follow-up medical attention stated that they did not receive any post-discharge contact.
